Use this form to declare the cancellation of your Streamtik subscription. You will state the date on which you would like the contract to end. You do not need to log in. After you submit the form you will receive a confirmation of receipt on this page.

Here you state the date you would like. We will tell you by email which date takes effect under the contract: during the minimum term the contract ends when that term expires, afterwards at the end of the current billing month.

To protect against automated submissions, Cloudflare Turnstile checks at submit time whether a person is filling in the form. Cloudflare receives your IP address and technical details of your browser for this. Details are in the privacy policy, section 3.6. If the check does not load for you, you can still submit the form.
